I authorize First Community Bank to initiate a single one-time debit for my initial deposit amount and micro-entries in amounts less than $1.00, if needed for account verification, to the Depository Financial Institution and account identified in this application. The origination of ACH transactions to my checking or savings account must comply with the laws of the United States of America. I represent and warrant that I am legally authorized to use the bank account specified.
If after submitting this authorization I choose to revoke this initial deposit debit to open an account with First Community Bank, if micro-entries are used for account verification, I will not complete the verification process detailed in the account application. If online account verification is used, processing of the initial deposit debit to open an account will begin on the day that immediate verification is completed and it may not be possible to revoke my authorization.
